Disaster Damage Certificate for Businesses 被災証明書(事業者)

Disaster certificates are also issued to businesses that incur damage due to natural disasters (storms, heavy rain, floods, tsunamis, earthquakes etc.) and fires, as proof of damage.

*Unlike the disaster victim certificate, it does not serve as proof of the degree or cost of damage.

Who is Eligible

Business owners who have incurred damage should apply in person. In the event a representative applies on their behalf, please submit the “Authorization Letter” found below together with the application form.

How to Apply

Please prepare the application form and supplementary documents listed below, and submit to the city hall (Commerce, Industry and Labour Division 商工労働課) via post or in person.

• Disaster Damage Certificate Application Form (Wordファイル:18.9KB)                             
• Authorization Letter (Wordファイル:20.1KB) (if a representative is applying on behalf)
• Photos of the damage *provide both full views and close-ups of the damaged portions.

Disaster Victim Certificate for House Damage

Those who have suffered damage to their house of residence can also apply for the Disaster Victim Certificate.
