For COVID-19 Vaccinations From 1 April 2024
Full public funding for COVID-19 vaccinations have ended as of 31 Mar 2024.
From 1 Apr 2024 onwards, vaccinations will be offered periodically from autumn to winter, for those 65 years old and above, as well as those eligible between 60 to 64 years old (see "Eligible Recipients" below). These will be provided for the purpose of preventing serious symptoms, and as a general rule be charged.
Vaccination outside the schedule periods will also be counted as self-voluntary vaccination be charged the full amount.
Scheduled Vaccination Periods for 2024
Purpose of Vaccination
Prevention of serious symptoms.
Eligible Recipients
- Those 65 years old and above
- Those 60 to 64 years old, deemed at high risk of serious symptoms
- With heart, kidney or respiratory disorders that severely constrain daily mobility
- Immunodeficiency due to HIV, essentially preventing normal daily living.
(NOTE)
- Those not fulfilling the above conditions can still vaccinate self-voluntarily at full cost.
- There is no obligation, nor advisories from local governments to vaccinate.
Vaccination Period
Around autumn to winter (exact period to be announced)
(NOTE) Any vaccination before the scheduled autumn-winter vaccination period will be charged at full cost as self-voluntary vaccination.

What to Bring for Vaccination
- Vaccination Coupon cum Pre-vaccination Questionnaire
- Fee stated in 1.
- Personal ID (health insurance card, driver's license, My Number Card etc.)
COVID-19 Certificate of Vaccination
Certificate of vaccination for COVID-19 vaccinations up until 2023 can be issued if required.
Who Is Eligible?
Those who fulfil both of the following conditions:
- Existing plans to travel overseas.
- Received COVID-19 vaccine while registered as a resident of Komatsu City.
Required Documents
- Application form
(can be obtained at service desk in Sukoyaka Center, or downloaded here:
Application form for COVID-19 Vaccination Certificate(PDFファイル:51.1KB)) - Copy of valid passport (page showing passport number)
- Copy of “Certificate of Vaccination for COVID-19” 予防接種済証 or “Record of Vaccination for COVID-19” 接種記録書 received upon vaccination.
- Document reflecting your vaccine coupon number (e.g “pre-vaccination screening only” 予診のみ section leftover from vaccine coupon)
- If applying via post, a return envelope (please include your address and name as well as the postage fee of 84 yen).
